How does the trial work?
Start the free 7-day trial in Form through Apple’s purchase sheet. After seven days, saving and editing require a one-time unlock. The US price is $0.99; the app shows the price for your App Store region. There is no subscription or automatic charge. Existing entries remain readable, comparable and exportable after the trial.
Restore a purchase
Open Settings → Restore purchases while signed into the same Apple Account used for the purchase. Restoring access does not restore journal photos or entries; use your Form ZIP backup or an Apple device backup for your data.
Add or change an entry
Use Log today or Edit today on Today. In History, select an entry to edit or delete it, or use the plus button to choose a past date. Weight and photos are optional individually; save at least a weight or a photo. Front, side and back photos can be recorded separately.
Compare photos
Open Compare and choose two dates with a photo from the same angle. Drag the divider or switch to Side by side. Form displays your saved images and does not reshape bodies or automatically align photos. The camera guide can help you repeat your framing.
Export or import a backup
In Settings, Export photos and data creates a ZIP containing your photos, weights and notes. Save it in Files. On another installation, choose Import backup and select the original Form ZIP. Review the counts and choose Keep existing or Replace existing for dates already in your journal. CSV export contains weights and notes; CSV import and third-party ZIP formats are not supported. The trial or unlock must be active to import.
Camera or photo access
If camera access was declined, enable it in iPhone Settings → Apps → Form. In the camera, take a photo and choose Use photo or Retake. A 3- or 10-second timer is available. Each angle has its own pinned guide: the first saved photo is selected initially. Tap the guide date to choose another photo from that angle, then adjust the opacity slider. The reference and opacity are remembered separately for Front, Side and Back. If an angle has no saved photo, its guide is unavailable. The guide is not added to your saved image. You can also use Library to select an existing photo. Photos stored only in iCloud may need to download before selection finishes.
Delete or protect your journal
Delete an entry in History, or clear the journal in Settings. The optional Face ID / device lock uses your iPhone’s authentication. A device passcode is required. Exports and original Photos images remain separate copies.
